Getting started: *(We are no longer accepting paper 应用程序s). All documentation will need to be uploaded to the online 应用程序. (See link to the left) Ensure all *required documentation is ready to upload before starting the
应用程序. (Application does not save and must be completed and submitted in one sitting.)
*Required Documentation:
Current TB test (within last 60 days) AND valid CA DL with clear photo
If uploaded Government Issued ID (CA DL) and home address on the VIPS 应用程序 do not match, please provide any of the following to verify your address: Voter's
Registration, paystub (last 30 days), utility bill (no cellular, cable or bank statement)
